Description
Seen from a dorsal (posterior-superior) angle at the ankle, the inferior extensor retinaculum forms a Y-shaped condensation of deep fascia spanning the anterior aspect of the tibiotalar region. Its stem arises laterally near the sinus tarsi and superior surface of the calcaneus, then divides into limbs that sweep medially to blend with the medial malleolar fascia and the plantar aponeurosis, crossing superficial to the extensor tendons. Deep to this band lie the tendons of tibialis anterior, extensor hallucis longus, and extensor digitorum longus as they pass toward the dorsum of the foot, with the extensor digitorum brevis muscle belly more distal and lateral. Orientation matters here because the inferior extensor retinaculum is the principal pulley that prevents bowstringing of the anterior compartment tendons during ankle dorsiflexion, and its septa organize the extensor tendon compartments where friction and stenosing tenosynovitis can occur after overuse or post-traumatic scarring. Surgeons also use the Y configuration as a surface landmark during anterolateral approaches to the ankle and hindfoot, where the deep fibular (peroneal) nerve and anterior tibial artery (dorsalis pedis continuation) run deep between extensor hallucis longus and extensor digitorum longus. Small structure, big consequences.
