Description
Spanning the anterior ankle, the inferior extensor retinaculum appears as a stout, Y-shaped fascial thickening anchored laterally near the calcaneus and fanning medially across the dorsum of the foot. Deep to its proximal limb lie the distal tibia and fibula forming the ankle mortise around the trochlea of the talus, with the retinacular bands crossing anterior to the tibiotalar joint line. Medially, the limbs course toward the medial malleolus and plantar aponeurosis region, while distally they extend toward the tarsus and metatarsals, forming fibrous tunnels that constrain the extensor tendons on their way to the toes. Bone landmarks remain cleanly readable. Functionally, this is the main checkrein preventing bowstringing of extensor digitorum longus and extensor hallucis longus during ankle dorsiflexion, and it establishes the osteofibrous compartments that guide tendon excursion and synovial sheath anatomy. It matters in anterior ankle approaches and arthroscopy portal planning, where incision placement and retraction must respect the extensor tendons and the deep fibular (peroneal) nerve with the dorsalis pedis artery as they pass on the dorsum, typically between extensor hallucis longus and extensor digitorum longus. Swelling or scarring here can contribute to anterior tarsal tunnel syndrome, producing dorsal foot pain and paresthesia in the first web space. A tight band.
