Description
Seen from the lateral thoracic wall, the external intercostal muscles occupy the intercostal spaces between adjacent ribs, with their fibers coursing infero-anteriorly from the inferior border of the rib above to the superior border of the rib below. Posteriorly, the muscle bellies approach the rib angles and relate closely to the thoracic vertebrae and costotransverse region, while anteriorly they taper toward the costochondral junction where the external intercostal membrane continues to the sternum. Superior ribs sit cranial to the iliac crest, and the upper pelvis anchors the frame of reference inferiorly. External intercostals are the workhorse layer for teaching thoracic wall mechanics because their fiber direction explains why they elevate ribs and expand the thoracic cage during inspiration, in contrast to the internal intercostals whose interosseous portion depresses ribs during forced expiration. Palpation and pain localization often follow these intercostal spaces.
