Seen from a lateral angle on a male forearm, the flexor carpi radialis is isolated along the anterior (flexor) compartment, running distal from the medial epicondyle region of the humerus toward the radial side of the wrist. The radius forms the lateral bony column, with the ulna positioned medial, and the elbow joint is defined by the distal humerus articulating with the proximal ulna and radius. Distally, the tendon of flexor carpi radialis courses toward the base of the second and third metacarpals, passing anterior to the carpus. Orientation matters. A lateral perspective clarifies why flexor carpi radialis can be palpated just lateral to the palmaris longus (when present) and why it remains outside the carpal tunnel, traveling in its own fibro-osseous groove at the level of the trapezium. That relationship helps when teaching wrist flexion with radial deviation, and it also frames common pain generators such as flexor carpi radialis tendinopathy near the volar-radial wrist, often confused clinically with de Quervain tenosynovitis at the radial styloid. Good landmarks. Better exams.
