Description
Running along the anterior forearm, the flexor carpi radialis muscle belly lies in the superficial flexor compartment, medial to the brachioradialis and lateral to the flexor digitorum superficialis as it courses distally toward the wrist. From this medial perspective, the radius remains lateral (thumb side) and the ulna medial, with the humerus proximal at the elbow and the carpal bones, metacarpals, and phalanges extending distally into the hand. A long, pale flexor carpi radialis tendon continues toward its insertion on the bases of the second and often third metacarpals, passing superficial to the flexor retinaculum rather than through the carpal tunnel. Orientation is unambiguous. Clinically, this muscle-tendon unit matters because the flexor carpi radialis tendon is a reliable surface landmark at the volar wrist, used to guide palpation of the radial artery just lateral to it and to orient volar approaches to the distal radius. Its close relationship to the median nerve and the flexor pollicis longus in the proximal forearm also makes it a reference point when teaching the layered dissection of the anterior compartment and when discussing pain generators such as flexor tendinopathy or overuse-related wrist flexion strain. Surgeons also harvest the flexor carpi radialis tendon as a graft in certain hand and wrist reconstructions. Clean anatomy helps reduce wrong-plane thinking.
