Description
Seen from the medial aspect of a pronated male upper limb, the extensor carpi radialis brevis (ECRB) is isolated in red along the proximal dorsolateral forearm, arising from the lateral epicondyle of the humerus via the common extensor tendon. From that proximal attachment, its muscle belly runs distally along the posterior radius, deep to extensor carpi radialis longus and extensor digitorum, then narrows into a tendon that courses toward the dorsal wrist. The radius lies lateral to the ulna in anatomic position, and the ECRB tracks nearer the radius, ending at the base of the third metacarpal on the dorsum of the hand. Bone landmarks are clear. Lateral epicondyle anatomy matters here because ECRB is the structure most often implicated in lateral epicondylitis, where microtearing and tendinosis develop at the common extensor origin and radiate pain just distal to the epicondyle with resisted wrist extension or gripping. A medial vantage also helps teach the layered relationship between ECRB, the supinator, and the radial nerve branches in the proximal forearm, an anatomy that often enters the differential when radial tunnel syndrome mimics tennis elbow. Palpation and injection accuracy depend on these spatial cues.
