Description
Oriented from the medial side of a male right upper limb, the extensor carpi ulnaris (ECU) is highlighted along the posteromedial forearm, arising near the lateral epicondyle of the humerus via the common extensor origin and continuing distally toward the ulnar aspect of the wrist. The muscle belly tapers into a long tendon that tracks posterior to the distal ulna and then courses toward the base of the fifth metacarpal, lying superficial to the ulna and medial to the radius as the two forearm bones converge at the wrist. Bony context includes the humerus at the elbow, the radius and ulna through the forearm, and the carpal, metacarpal, and phalangeal elements in the hand. Relationship is the message. Clinically, this medial perspective is a practical way to teach why the ECU behaves differently from the flexor carpi ulnaris, despite their shared ulnar-sided destination. The ECU tendon passes through the sixth dorsal compartment and is stabilized by a fibro-osseous subsheath at the ulnar groove, a setup that predisposes it to ulnar-sided wrist pain from ECU tendinopathy or tendon subluxation during forearm supination with resisted wrist extension and ulnar deviation. Distal to the ulnar head, the tendon’s position also helps explain exam maneuvers used to localize pain to the ECU versus the triangular fibrocartilage complex (TFCC). Small distinctions matter at the wrist.
