Description
Laterally on the proximal forearm, the extensor carpi radialis brevis (ECRB) lies deep to extensor carpi radialis longus and anterior to extensor digitorum, with its short, dense muscle belly arising from the lateral epicondyle of the humerus via the common extensor tendon. Distally, its tendon tracks along the dorsolateral wrist, passing beneath the extensor retinaculum in the second dorsal compartment alongside the extensor carpi radialis longus tendon. The muscle mass sits superficial to the radial head and neck region, with the brachioradialis forming a more anterior, lateral contour. Clean landmarks. Clinically, ECRB is the primary pathologic tendon in lateral epicondylitis (tennis elbow), where degenerative change often localizes near its origin at the lateral epicondyle rather than a true inflammatory tendinitis. This lateral perspective helps separate ECRB from extensor carpi ulnaris and the extensor digitorum tendons during physical examination maneuvers and during a lateral approach for debridement or epicondylar release, where preserving adjacent structures such as the radial collateral ligament complex matters. It also supports teaching why resisted wrist extension with the elbow extended can reproduce pain when the ECRB origin is involved.
