Description
Running along the lateral aspect of the posterior forearm, the extensor carpi radialis brevis lies deep and slightly medial to the extensor carpi radialis longus, its fusiform belly tapering into a tendon as it courses distally toward the dorsum of the wrist. Proximally, the muscle arises from the lateral epicondyle of the humerus via the common extensor origin and from adjacent radial collateral ligament fibers, then passes superficial to the radius and deep to the overlying skin and superficial fascia. Distally, its tendon tracks beside the extensor carpi radialis longus through the second dorsal compartment beneath the extensor retinaculum, angling to its insertion at the dorsal base of the third metacarpal, with the radius, ulna, carpal bones, metacarpals, and phalanges providing clear bony landmarks in profile. Spatial relationships are clean in a lateral view. Clinically, extensor carpi radialis brevis is the workhorse lesion in lateral epicondylitis, where degenerative tendinopathy concentrates at its origin on the lateral epicondyle and can be provoked by resisted wrist extension with the elbow extended. The lateral perspective helps distinguish brevis from longus by its shorter muscle belly and more central line of pull to the third metacarpal, a distinction that matters during surgical debridement for recalcitrant tennis elbow and when teaching why grip strength drops as the wrist falls into flexion. Note the neighborhood. The superficial branch of the radial nerve and the radial artery course nearby at the distal radial forearm, so this orientation supports safe surface and operative planning.
