Description
Positioned on the posterolateral forearm, the extensor carpi radialis brevis lies deep to the skin and superficial fascia, its muscle belly sitting medial to extensor carpi radialis longus and just posterior to brachioradialis as the radial border of the dorsal compartment comes into view. Distally, its tendon narrows and courses under the extensor retinaculum in the second dorsal compartment, running alongside extensor carpi radialis longus before diverging to its insertion on the base of the third metacarpal. More medially on the dorsum, extensor digitorum and extensor carpi ulnaris occupy the central and ulnar aspects of the posterior forearm, with their tendons tracking toward the extended fingers. Bony landmarks of the hand are faintly apparent beneath the soft tissues. Clear orientation. Clinically, extensor carpi radialis brevis is the usual culprit in lateral epicondylitis, since its common extensor origin at the lateral epicondyle is exposed to repetitive wrist extension and gripping loads, and posterior views like this make the muscle’s relationship to extensor carpi radialis longus and the extensor digitorum mass easier to teach. The course of the extensor tendons toward the wrist also supports discussion of the extensor retinaculum, dorsal compartments, and why tenosynovitis and post-traumatic adhesions can limit wrist extension even when finger extension appears preserved.
