Description
Posterior forearm anatomy is presented with the extensor carpi radialis brevis (ECRB) coursing along the lateral (radial) compartment beneath the skin of an adult male. Proximally, the muscle belly lies deep to the extensor carpi radialis longus and superficial to the supinator, taking origin from the lateral epicondyle region and blending with the common extensor tendon. Distally, its tendon runs toward the dorsum of the wrist, passing in line with the radius (lateral) and remaining separate from the more medial extensor digitorum tendons as the ulna tracks medially. Bony landmarks include the radius and ulna, distal radioulnar region, and the carpal and metacarpal framework in dorsal projection. Clear landmarks. ECRB matters because it is the structure most often implicated in lateral epicondylitis, where degenerative tendinopathy occurs near its origin at the lateral epicondyle and adjacent common extensor tendon, a frequent source of lateral elbow pain in racquet sports and repetitive wrist extension work. The posterior orientation helps clarify why provocative tests that load wrist extension with the elbow extended can reproduce symptoms, and it supports procedural planning for corticosteroid injection, needle tenotomy, or surgical débridement while respecting the nearby radial nerve branches in the lateral elbow. It also reinforces the functional line of pull: wrist extension with a radial deviation bias.
