From a lateral viewpoint of the male posterior hip, gluteus maximus forms the most superficial mass of the buttock, its oblique fibers coursing inferolaterally from the posterior ilium, sacrum, and sacrotuberous ligament toward the iliotibial tract and gluteal tuberosity of the femur. Superior and slightly anterior to it, gluteus medius lies as a fan-shaped abductor over the lateral ilium, converging on the greater trochanter. Along the lateral thigh you can expect the iliotibial band tracking distal toward the lateral femoral condyle, with vastus lateralis anteriorly and biceps femoris posteriorly framing the proximal femur. Lateral anatomy here matters because many posterior hip exposures split gluteus maximus in line with its fibers to reach the short external rotators and hip capsule, and the nearby sciatic nerve remains a constant concern as it exits inferior to piriformis and travels deep to the maximus. Injection technique also depends on these surface relationships: the safe intramuscular site targets the superolateral quadrant to avoid the sciatic nerve and the inferior gluteal vessels. When gluteus medius is emphasized, the view also supports teaching pelvic stability, since weakness or superior gluteal nerve injury produces a Trendelenburg gait with contralateral pelvic drop.
