Description
Superficial skin and subcutaneous tissue are removed to expose the paired gluteus maximus muscles forming the bulk of the posterior buttock. Each muscle arises broadly from the posterior ilium and dorsal surface of the sacrum and coccyx, then runs inferolaterally, with upper fibers converging into the iliotibial tract along the lateral thigh and deeper lower fibers inserting on the gluteal tuberosity of the proximal femur. Superior to the gluteal mass the lower lumbar vertebrae and lumbosacral junction provide bony context, while the iliac crests sit laterally and the proximal femora lie inferior and slightly lateral to the pelvis. Fiber direction is clear. Gluteus maximus is the primary hip extensor and an external rotator, and its insertion into the iliotibial tract explains how posterior pelvic mechanics influence lateral knee tracking during gait and running. This posterior superficial view is also the one clinicians default to when planning safe intramuscular injection sites, because the superolateral quadrant is chosen to avoid the sciatic nerve and major gluteal vessels that course deep to the muscle. Posterior hip approaches, gluteus maximus splitting, and reconstruction using a gluteus maximus flap all depend on understanding the muscle’s broad origin and the way its fascial attachment blends into the iliotibial band.
