Description
Seen in lateral profile, the levator anguli oris (also referred to in older texts as the levator anguli oris or quadratus labii) rises from the canine fossa of the maxilla, just inferior to the infraorbital foramen, and courses inferolaterally toward the modiolus at the angle of the mouth. Anterior to it lie the fibers of levator labii superioris and the nasal musculature near the alar cartilage, while posteriorly the zygomaticus major and minor approach the upper lip and commissure from the zygomatic bone. Deep to the highlighted elevator, the buccinator forms the muscular wall of the cheek, with the mandible and maxillary dentition providing rigid landmarks. Small but specific. Functionally, this muscle is one of the direct elevators of the oral commissure, acting with zygomaticus major during smiling and counterbalancing depressor anguli oris during expressions of sadness or fatigue. A lateral dissection-style view is practical because it clarifies how the levator anguli oris sits anterior to the masseteric region yet remains tethered to the maxilla, a relationship that matters when interpreting asymmetry in facial nerve palsy or planning perioral reconstruction. Buccal space infections, intraoral incisions, and suturing around the modiolus all benefit from a clear mental map of where this elevator crosses superficial planes relative to buccinator and the nasolabial fold.
