Description
Arising from the canine fossa of the anterior maxilla, the levator anguli oris ascends superiorly and slightly laterally to blend with the modiolus at the angle of the mouth, deep to the superficial mimetic muscles. In lateral perspective, its belly sits inferior to the infraorbital margin and adjacent to the maxillary sinus region, with the zygomaticus major and minor running more obliquely from the zygomatic bone toward the upper lip and oral commissure. Posteriorly, the buccinator forms the muscular wall of the cheek and meets the orbicularis oris at the modiolus, creating a crowded intersection of fibers where subtle changes in direction matter. Bony landmarks such as the maxilla, zygoma, mandible, and dentition frame the soft tissue attachments. Clinically, the levator anguli oris helps elevate the oral commissure, so its relationship to the modiolus is central when teaching perioral expression mechanics and when planning aesthetic or reconstructive work around the smile line. The infraorbital foramen lies superior and slightly lateral to its origin, a practical warning zone during maxillofacial approaches and local anesthetic blocks, and edema or scarring in the canine fossa can tether its function.
