Description
Laterally, the cervical contour is dominated by the sternocleidomastoid (SCM) coursing obliquely from the manubrium and medial clavicle to the mastoid process and lateral part of the superior nuchal line. Posterior to it, the splenius capitis would be expected to form a broad, straplike layer running superolaterally from the lower nuchal ligament and spinous processes of C7 to T3 or T4 toward the mastoid process and adjacent occipital bone, occupying the posterolateral neck behind the SCM. Deep to these planes sit the cervical vertebrae and the occiput, with the mandible and ribs providing bony reference for head and shoulder girdle orientation. Clear landmarks. A lateral perspective matters because it clarifies the functional opposition and cooperation between superficial rotation muscles, the SCM rotating the face to the contralateral side while splenius capitis rotates and laterally flexes the head ipsilaterally. Clinically, this relationship underpins bedside examination of acute torticollis and helps explain why posterior neck strain patterns often localize to the splenius region after sudden extension or rotation, while SCM spasm can present with a different head posture and palpable cordlike tension along the anterolateral neck. For procedural anatomy, the lateral neck is also where surface landmarks guide safe trajectories for cervical injections and surgical approaches, with the SCM defining compartments and masking deeper posterior muscles.
