Description
Seen in lateral profile beneath the skin, the splenius capitis occupies the posterolateral neck, running superomedially from the nuchal ligament and spinous processes of C7 to T3 toward the mastoid process and lateral third of the superior nuchal line. Anterior to it, the sternocleidomastoid forms a prominent strap from the mastoid to the manubrium and medial clavicle, creating a clear superficial boundary between anterior and posterior cervical compartments. Superiorly, the temporalis fans over the temporal fossa and the masseter overlies the ramus of the mandible, while the auricle and external acoustic meatus provide surface landmarks that help orient the muscle attachments at the skull base. Lateral views like this matter when teaching functional neck anatomy because splenius capitis acts as a strong extensor and ipsilateral rotator of the head, working with semispinalis capitis and upper trapezius while being opposed by the sternocleidomastoid during contralateral rotation. Palpation and symptom mapping can be confusing in patients with whiplash-associated disorders or cervical myofascial pain, where trigger points in splenius capitis often refer pain to the occiput, mastoid region, and behind the eye, mimicking cervicogenic headache or occipital neuralgia patterns. Surface relationships help. The muscle’s position deep to the superficial fascia also explains why edema or spasm can feel diffuse on exam.
