Description

Seen from a lateral perspective in an adult male, the superior gluteal vein tracks deep to gluteus maximus and courses superior to the piriformis as it follows the superior gluteal artery toward the greater sciatic foramen. Proximally, the vessel passes between gluteus medius and gluteus minimus, then turns medially toward the pelvis to drain into the internal iliac venous system. Small venules from the lateral gluteal musculature converge into the main trunk. Relationships to the posterior ilium, greater trochanter region, and adjacent sciatic notch contours are implied by the vessel’s trajectory. Appreciating the superior gluteal vein in this lateral orientation matters when you are teaching the anatomy of the greater sciatic foramen and the suprapiriform compartment, where the superior gluteal neurovascular bundle exits the pelvis and becomes vulnerable to iatrogenic injury. A clear view of the vein’s companion course with the artery helps contextualize hemorrhage risk during posterior or lateral hip exposure and the patterns of bleeding that can occur with pelvic fractures involving the greater sciatic notch. It also supports understanding of safe intramuscular injection zones in the superolateral buttock, where deep vessel proximity is often underappreciated. Small details, big consequences. Common use cases include pelvic and gluteal region anatomy teaching in gross anatomy and surgical anatomy courses, figure support for orthopedic and trauma chapters covering posterior hip approaches and pelvic ring injuries, and clinical education materials on gluteal injections and superior gluteal vessel injury patterns.
