Running along the posterior ilium above the piriformis, the superior gluteal vein (and its accompanying venules) emerges from the deep gluteal compartment to join the superior gluteal vessels as they course laterally between gluteus medius and gluteus minimus. Superficial to the vessel, gluteus maximus frames the buttock, while the sacrum and posterior iliac crest sit superior and medial, and the acetabulum and proximal femur lie inferolateral. Deeper lateral rotators, including obturator internus and the superior and inferior gemelli, sit inferior to the venous trunk near the greater sciatic foramen. Posterior gluteal venous anatomy matters when you are teaching the suprapiriform and infrapiriform corridors of the greater sciatic foramen and their contents.
