Description

Posteriorly, the pancreas lies transversely across the upper abdomen, with its head nested in the C loop of the duodenum and its body and tail extending leftward toward the splenic hilum. An inferior pancreatic vein courses along the inferior margin of the pancreatic body, receiving small venules from the pancreatic parenchyma and passing posterior to the stomach, anterior to the aorta, and in close relationship to the superior mesenteric vessels. Nearby, the splenic vein typically runs along the posterior surface of the pancreas, joining the superior mesenteric vein to form the hepatic portal vein superior and to the right of the pancreatic neck. Blue venous channels from the mesentery converge inferiorly, while red arterial anatomy includes the abdominal aorta and its major branches. A posterior angle is where pancreatic venous drainage becomes legible, because the dominant outflow pathways, including the splenic vein, superior mesenteric vein, and smaller tributaries such as the inferior pancreatic vein, sit behind the gland and can be obscured in anterior teaching views. This relationship matters in pancreaticoduodenectomy and distal pancreatectomy, where control of pancreatic venous tributaries reduces back-bleeding and helps prevent injury to the porto-mesenteric confluence. It also grounds interpretation of portal hypertension patterns, since elevated portal pressures can distend pancreatic and gastroepiploic tributaries that communicate with the splenic and superior mesenteric venous systems.
