Description
Arising from the external occipital protuberance, medial superior nuchal line, and the nuchal ligament, the trapezius fans inferiorly to the spinous processes of the lower cervical and thoracic vertebrae, then sweeps laterally toward the shoulder girdle. In lateral perspective, its superior fibers course anterolaterally toward the lateral third of the clavicle, the middle fibers run more horizontally to the acromion and spine of the scapula, and the inferior fibers ascend superolaterally from the lower thoracic midline to the medial end of the scapular spine. Deep and anterior to the muscle belly, the rib cage and vertebral column establish the thoracic contour, with the thoracolumbar junction and posterior iliac crest inferiorly framing the lower attachment region. This angle matters because it ties trapezius fiber direction to scapular kinematics: upper and lower fibers couple to produce upward rotation, while the middle fibers stabilize scapular retraction against protraction forces. Clinically, that relationship underpins common patterns after spinal accessory nerve (CN XI) injury in posterior triangle surgery or cervical lymph node biopsy, where lateral shoulder droop and impaired overhead elevation reflect trapezius weakness, often with secondary overactivity of levator scapulae and serratus anterior dyskinesis.
