Running across the lateral face, the zygomatic branch of the facial nerve (CN VII) emerges from the parotid region and sends several rami anteriorly and superiorly toward the lateral canthus and infraorbital area. Superior to it sit the zygomatic bone and the frontal process of the maxilla, while posteriorly the temporal bone and external acoustic meatus anchor the otic region; inferiorly, the mandible and dentition define the lower facial framework. Along the cheek, these motor fibers course in the superficial musculoaponeurotic plane, generally superficial to the masseter and deep to the skin, with neighboring sensory territory supplied by the maxillary division of the trigeminal nerve (CN V2) via the zygomaticofacial and infraorbital branches. Clinically, this is the branch surgeons worry about when operating near the zygomatic arch and lateral orbit, because injury can denervate orbicularis oculi and contribute to lagophthalmos with exposure keratopathy. Its trajectory also explains why parotidectomy, facelift (rhytidectomy), and repair of zygomaticomaxillary complex fractures demand careful respect for the anterior parotid border and SMAS dissection planes.
