Description
Along the lateral border of the plantar foot, the abductor digiti minimi is visible beneath the skin as a long fusiform belly extending from the calcaneal tuberosity toward the base of the proximal phalanx of the fifth toe. Its fibers course anteriorly and slightly distally along the lateral pedal margin, superficial to the lateral plantar neurovascular plane and adjacent medially to the flexor digitorum brevis. Posteriorly, the relationship to the calcaneus and the plantar fascia is apparent, while distally the muscle narrows toward its tendon near the fifth metatarsophalangeal region. Bone landmarks in this lateral view logically include the calcaneus, cuboid, fifth metatarsal, and the phalanges, with the Achilles tendon inserting on the posterior calcaneus. A lateral perspective matters because it clarifies how the lateral compartment of the first plantar layer anchors to the calcaneus and stabilizes the fifth ray during stance and push-off. Pain along the lateral plantar heel often gets labeled as plantar fasciitis, but irritation at the abductor digiti minimi origin and along the course of the lateral plantar nerve can produce a similar clinical picture, including Baxter nerve entrapment between the abductor hallucis and quadratus plantae.
