Description
Arising from the medial and lateral processes of the calcaneal tuberosity and the plantar aponeurosis, the abductor digiti minimi forms a broad, fleshy belly along the lateral border of the sole. From proximal to distal it courses anteriorly, superficial to the quadratus plantae and lateral to the flexor digitorum brevis, then narrows toward the fifth ray. The tendon passes to the lateral side of the base of the proximal phalanx of the fifth toe, adjacent to the fifth metatarsal head at the metatarsophalangeal joint. Bone landmarks from the phalanges through the metatarsals and into the tarsus, including the cuboid and lateral cuneiform region, frame the muscle’s line of pull. Functionally, abductor digiti minimi provides active abduction of the fifth digit and helps stabilize the lateral longitudinal arch during late stance, when the forefoot loads and the plantar soft tissues tension. Clinically, this muscle belongs to the first (superficial) plantar layer and is a key landmark when teaching plantar compartments or planning a lateral plantar approach to the cuboid or calcaneus. It also helps orient learners to the course of the lateral plantar neurovascular bundle, which runs deep to this layer and can be at risk during plantar fasciotomy or incision placement along the lateral sole.
