Description
Running along the lateral forearm, the extensor carpi radialis longus (ECRL) sits just posterior to the brachioradialis, emerging from the lateral supracondylar ridge of the humerus and tapering into a long distal tendon. From this lateral perspective under the skin, the muscle belly occupies the proximal radial side, while its tendon tracks distally toward the dorsum of the wrist, staying lateral to the bulk of extensor digitorum and extensor carpi ulnaris. Deep and slightly posterior, extensor carpi radialis brevis often parallels it, and the two tendons pass beneath the extensor retinaculum in the second dorsal compartment en route to their insertions at the bases of the second and third metacarpals, respectively. Clear surface anatomy. ECRL matters because it is a prime wrist extensor and radial deviator, and its proximal attachment near the lateral epicondylar region makes it clinically entangled with lateral elbow pain patterns and the differential for so-called lateral epicondylitis. Its course also brings it into practical proximity with the superficial branch of the radial nerve and the radial artery in the distal forearm, which is why this lateral dissection-style view works well when teaching safe corridors for injections, surgical exposure, or tendon harvest. Tendon friction at the dorsal wrist, including intersection-type overuse syndromes, is easier to explain when the second compartment anatomy is explicit.
