Description

Running along the lateral aspect of the right leg, the fibularis (peroneus) brevis sits deep to the crural fascia and superficial to the distal fibula, its muscle belly arising from the inferior two-thirds of the fibular shaft. Anterior to it lie the fibularis longus and the anterior intermuscular septum bordering the extensor compartment, while posteriorly the soleus and the posterior intermuscular septum define the back of the lateral compartment. Distally, the brevis tendon narrows and courses posterior to the lateral malleolus in the retromalleolar groove, held in place by the superior and inferior fibular (peroneal) retinacula before turning toward the base of the fifth metatarsal. Skin remains intact. Clinically, isolating fibularis brevis in a lateral subdermal view clarifies why lateral ankle sprains often travel with peroneal tendon pain, tenosynovitis, or retinacular injury, since the tendon is tethered behind the lateral malleolus where inversion forces concentrate. The distal insertion explains the mechanism of a tuberosity avulsion fracture of the fifth metatarsal, in which traction from fibularis brevis can displace the fragment and mimic or accompany so-called Jones region fractures. You also get a clean teaching angle on lateral compartment syndrome and the nearby superficial fibular nerve, which becomes symptomatic when swelling tightens the fascia. Orthopaedic and sports medicine texts use this perspective to illustrate peroneal tendon subluxation, retinaculum repair, and landmark-based palpation along the distal fibula and lateral foot.
