Description
Running along the lateral aspect of the leg, the fibularis brevis (peroneus brevis) lies in the lateral compartment, deep and slightly anterior to fibularis longus as it tracks inferiorly beside the fibular shaft. Its short, thick muscle belly occupies the distal half of the fibula, then narrows into a tendon that passes posterior to the lateral malleolus and courses toward the base of the fifth metatarsal. Posteriorly, the gastrocnemius and soleus form the superficial calf, while anteriorly the tibialis anterior and extensor group sit across the tibial crest and anterolateral leg. Bones and tendons orient the view. Clinically, this lateral perspective clarifies the peroneal groove behind the lateral malleolus, where fibularis brevis shares a synovial sheath with fibularis longus and is stabilized by the superior and inferior fibular retinacula. Tears and split lesions of fibularis brevis often occur at this level, sometimes with retinacular injury and tendon subluxation after an inversion ankle sprain, and the illustration helps relate symptoms to the tendon’s retromalleolar course. The distal insertion on the fifth metatarsal also frames the differential with an avulsion fracture of the tuberosity and with painful osseous variants near the lateral midfoot.
