Description
Running along the lateral compartment of the leg, the fibularis (peroneus) longus forms a thick proximal muscle belly adjacent to the head and lateral shaft of the fibula, then narrows into a long tendon that passes posterior to the lateral malleolus before continuing onto the plantar foot. From this lateral left hemibody perspective, you also expect to see the fibularis brevis lying deep and slightly anterior to longus distally, the fibular shaft as the bony landmark, and the anterior compartment nearby with tibialis anterior more anteromedial. Posteriorly, the gastrocnemius and soleus bulk frame the Achilles tendon as it descends to the calcaneus. Clear compartmental topography. Clinically, this view helps orient the peroneal tendons in relation to the fibula and lateral malleolus, the corridor where tendinopathy, longitudinal split tears (often in fibularis brevis), and retinacular injury can lead to tendon subluxation. Palpation and resisted eversion testing track the fibularis longus from its proximal belly to the retromalleolar groove, and surgeons use these relationships during lateral ankle approaches and peroneal tendon repairs. The longus tendon’s plantar course also ties directly to first-ray stabilization and arch mechanics, relevant in cavovarus foot and chronic lateral ankle instability. Use this asset in lower-limb anatomy and kinesiology teaching to contrast the lateral compartment with the anterior and posterior compartments, and in sports medicine or orthopedic publications discussing peroneal tendon pathology and lateral ankle sprain sequelae.
