Description
Oriented in an anterior view of the male lower limb, the fibularis (peroneus) longus occupies the lateral compartment of the leg, its long fusiform belly tracking inferiorly along the proximal fibula. Medially, the tibial crest and anterior border of the tibia define the shin, while the fibula remains lateral and slightly posterior, giving the muscle its clear lateral profile. Distally, the fibularis longus tendon narrows and courses toward the lateral malleolus, passing posterior to it before turning onto the plantar aspect of the foot to reach the base of the first metatarsal and medial cuneiform. Bones of the knee, leg, and foot provide landmarks, including the distal femur, patella, tibia, fibula, tarsals, metatarsals, and phalanges. Functionally, this is the primary evertor of the foot and a strong plantarflexor assistant, and its plantar course creates a stirrup effect with tibialis anterior that supports the transverse arch. Clinically, the tendon’s relationship to the retromalleolar groove and the superior and inferior fibular retinacula explains common pain generators in lateral ankle overuse, including peroneal tendinopathy, tenosynovitis, and tendon subluxation after inversion injuries. A clean anterior-lateral view also helps distinguish lateral compartment anatomy from flexor digitorum longus, which lies in the deep posterior compartment and runs behind the medial malleolus.
