Description
Running along the lateral leg just deep to the skin, the fibularis (peroneus) longus belly occupies the superficial lateral compartment, lying lateral to the fibula and anterior to the posterior compartment bulk of the gastrocnemius and soleus. Distally, its tendon narrows and tracks posterior to the lateral malleolus, sharing the retromalleolar groove with the fibularis brevis tendon, then courses inferiorly toward the plantar foot to reach the base of the first metatarsal and medial cuneiform. An anterior oblique angle also places the tibial crest and anterior compartment tendons (tibialis anterior and extensor digitorum longus) in relative medial view, framing the fibularis longus as the dominant lateral contour.
