Description
Seen from a posterior perspective, the fibularis (peroneus) longus occupies the lateral compartment of the male leg, arising from the head and proximal lateral surface of the fibula and tracking inferiorly toward the lateral malleolus. Medial to it, the tibia forms the dominant vertical column of the leg, while the fibula lies lateral and slightly posterior, providing the bony bed for the muscle’s proximal fibers. Superiorly, the knee region is included, with the distal femur articulating with the tibial plateau and the fibular head positioned posterolaterally. Posterior exposure clarifies how the fibularis longus tendon courses behind the lateral malleolus within the retromalleolar groove, sharing this tight corridor with the fibularis brevis and constrained by the superior and inferior fibular (peroneal) retinacula. That relationship matters when teaching or planning around lateral ankle pain: peroneal tendinopathy, longitudinal split tears, and tendon subluxation often follow inversion injuries, and palpation or surgical repair hinges on knowing what sits directly posterior to the distal fibula. It also frames the muscle’s functional tie-in to hindfoot eversion and first-ray plantarflexion as the tendon crosses the plantar foot to the base of the first metatarsal and medial cuneiform.
