Fibularis Longus

Description

Running along the lateral compartment of the male leg, the fibularis (peroneus) longus arises from the proximal fibula and adjacent intermuscular septa, then tapers into a long tendon that descends posterior to the lateral malleolus. From that retromalleolar groove, the tendon continues inferiorly into the foot, where it courses plantarward to cross the sole toward its insertion on the base of the first metatarsal and medial cuneiform. The fibula lies lateral to the tibia, and the fibularis longus tendon sits posterolateral to the ankle joint as it turns sharply around the malleolus. A tight corner. That change in direction behind the lateral malleolus is a common source of pain and instability, and this view clarifies why. Peroneal tendon subluxation or dislocation can follow injury to the superior fibular retinaculum, while chronic friction in the fibular (peroneal) groove and at the peroneal tubercle of the calcaneus contributes to tenosynovitis and longitudinal tendon tears. Surgeons planning retinacular repair, groove deepening, or peroneal tendon debridement need a clean mental map of the tendon’s posterolateral path and its relationship to the lateral malleolus, calcaneofibular ligament region, and distal fibula. Use this illustration in lower limb anatomy teaching to contrast the lateral compartment with tibialis anterior on the anterior leg, and to reinforce the mechanics of eversion and plantarflexion during gait.
