Description
Running along the lateral compartment of the male leg, the fibularis (peroneus) longus arises from the head and proximal two thirds of the lateral fibula and adjacent crural fascia, its muscle belly lying lateral and slightly posterior to the fibular shaft. Distally, the fibers converge into a thick rounded tendon that passes posterior to the lateral malleolus, held in place within the retromalleolar groove by the superior fibular (peroneal) retinaculum. From there the tendon courses plantar and medially across the sole in the cuboid groove to insert on the plantar aspect of the medial cuneiform and base of the first metatarsal, deep to the plantar aponeurosis and lateral to the long plantar ligament. This lateral-to-plantar trajectory makes the fibularis longus a common point of confusion in teaching because the tendon begins superficially at the ankle yet ends medially on the first ray, forming a functional sling with tibialis posterior that stabilizes the transverse arch. Clinically, the segment posterior to the lateral malleolus is where peroneal tendinopathy, tenosynovitis, and tendon subluxation occur after retinacular injury, and it is also the pathway at risk during lateral ankle approaches and fixation near the distal fibula. The cuboid tunnel is a second choke point, often implicated in plantar lateral foot pain and stenosing tenosynovitis.
