Description
Seen from the lateral aspect of an adult male hip, the gluteus maximus dominates the posterior buttock as a thick superficial sheet, its fibers running inferolaterally from the posterior ilium and sacrum toward the femur and iliotibial tract. Superior to its bulk, the iliac crest region and the posterolateral pelvis set the proximal origin field, while the muscle’s distal contour approaches the gluteal tuberosity and blends into the fascia lata laterally. Deep and anterior margins logically relate to the greater trochanter region, where the gluteus medius and minimus lie more anterosuperior, and the proximal hamstrings emerge inferiorly from the ischial region. Fiber direction is the story here. Functionally, this lateral view clarifies why gluteus maximus is the primary hip extensor and a major contributor to external rotation, and why its iliotibial tract insertion matters for lateral knee mechanics during gait and running. It also supports clinical teaching around common injection landmarks: the safe intramuscular target is superolateral in the gluteal region to avoid the sciatic nerve, which courses deep and inferior to the muscle toward the posterior thigh. Surgical planning benefits too, since posterior and posterolateral approaches to the hip must respect the gluteus maximus split and the relationship of its deep surface to the short external rotators.
