Description
Viewed laterally on a male pelvis, the gluteus medius occupies the outer surface of the ilium, arising between the anterior and posterior gluteal lines and converging inferiorly toward its tendon on the lateral facet of the greater trochanter. Superior to it sit the iliac crest and anterior superior iliac spine, while posteriorly it approaches the posterior superior iliac spine and the sacroiliac region. Anteriorly the tensor fasciae latae and iliotibial tract lie in close apposition, and inferiorly the proximal femur frames the muscle’s insertion. Deep to the medius, the gluteus minimus would be expected, with the gluteus maximus typically superficial but not the primary focus in this view. Clear landmarks. Functionally, this lateral perspective matters because it is the best way to teach the abductor mechanism that stabilizes the pelvis in single leg stance. Weakness of the gluteus medius, from L5 radiculopathy, superior gluteal nerve injury, or chronic tendinopathy at the greater trochanter, produces a Trendelenburg gait and often coexists with lateral hip pain labeled greater trochanteric pain syndrome. For surgeons, the relationship to the greater trochanter and proximal femur also aligns with the direct lateral (Hardinge) approach to the hip, where splitting the gluteus medius can risk abductor insufficiency if repair fails. Use this asset in gross anatomy and kinesiology modules on the gluteal region, in orthopaedic or sports medicine texts explaining hip abductor tears, and in patient facing materials for lateral hip pain and post hip arthroplasty rehabilitation.
