Description
Seen from a lateral perspective, the gluteus medius forms a broad, fan-shaped muscle over the lateral surface of the ilium, spanning from the area between the anterior and posterior gluteal lines toward its distal insertion on the lateral facet of the greater trochanter of the femur. Superiorly, the iliac crest provides the proximal bony border, while inferiorly the muscle tapers toward the hip joint capsule and proximal femur. The gluteus maximus is implied as the more superficial posterior muscle that normally overlies part of the gluteus medius, and the lumbar vertebrae and lumbosacral junction sit medial and superior to the acetabular region. Clear bony landmarks anchor the relationship. Hip abduction is only part of the story. During single-leg stance, anterior fibers of gluteus medius (with gluteus minimus) counter pelvic drop on the contralateral side, and loss of that function produces the classic Trendelenburg gait seen with superior gluteal nerve injury, L5 radiculopathy, or chronic tendinopathy at the greater trochanter. Surgeons and proceduralists also track this anatomy when planning a lateral or anterolateral approach to the hip and when localizing the superior gluteal neurovascular bundle, which courses deep to the gluteus medius above the piriformis. Use this plate in gross anatomy and kinesiology teaching to connect iliac surface anatomy to the abductor mechanism, or in orthopedic and sports medicine materials discussing greater trochanteric pain syndrome, gluteus medius tears, and postoperative abductor insufficiency after total hip arthroplasty.
