Description
Prominently highlighted on the lateral hip, the gluteus medius fans from the outer surface of the ilium, spanning between the anterior and posterior gluteal lines and inserting distally onto the lateral aspect of the greater trochanter of the femur. Deep to it, the gluteus minimus tracks from the ilium to the anterior facet of the greater trochanter, just superior to the hip joint capsule. Surrounding bony landmarks include the iliac crest superiorly, the acetabulum anteroinferiorly, and the sacrum and lower lumbar vertebrae medially, with the proximal femur oriented inferiorly and slightly anterior relative to the pelvis. Hip abduction is only part of the story. During single leg stance, gluteus medius and minimus generate an abductor moment that prevents contralateral pelvic drop, the biomechanical basis of the Trendelenburg sign and gait seen with superior gluteal nerve injury (L4 to S1) or chronic gluteal tendinopathy. Lateral hip pain is often misattributed to trochanteric bursitis, but a clear depiction of the medius insertion on the greater trochanter supports teaching the modern concept of greater trochanteric pain syndrome, where partial thickness tears and enthesopathy at the abductor footprint are common. This lateral relationship also matters in the direct lateral approach to total hip arthroplasty, where splitting the gluteus medius risks postoperative abductor weakness.
