Description
Positioned on the posterolateral ilium, the gluteus medius appears as a broad, fan-shaped muscle arising between the anterior and posterior gluteal lines and converging into a thick tendon that passes inferiorly to the lateral surface of the greater trochanter. Medially, the sacrum anchors the midline of the posterior pelvis, while the iliac crests frame the superior border of the scene. Inferior and medial to the iliac wing, the ischium contributes to the posterior pelvic ring, and the proximal femur is seen laterally as it articulates at the acetabulum. Bilateral symmetry is emphasized. Hip abductor mechanics are easiest to teach from a posterior viewpoint because you can track the muscle’s line of pull relative to the pelvis and greater trochanter and relate it directly to frontal-plane pelvic stability during single-leg stance. Weakness or denervation of the gluteus medius, classically from L5 radiculopathy or superior gluteal nerve injury, produces a Trendelenburg gait with contralateral pelvic drop. Surgeons also care about this tendon, since lateral and anterolateral approaches to the hip place the gluteus medius and minimus insertions at risk, and chronic lateral hip pain is often driven by gluteus medius tendinopathy or partial-thickness tearing at the trochanteric footprint.
