Description
Deep to the gluteus maximus and inferior to the iliac crest, the gluteus minimus occupies the lateral surface of the ilium, its fibers converging anteroinferiorly toward a tendon that inserts on the anterior facet of the greater trochanter of the femur. A posterior perspective places the muscle lateral to the posterior ilium and sacrum, with the gluteus medius lying superficial and slightly posterior, and the proximal vastus lateralis and tensor fasciae latae region sitting more distal and anterolateral around the hip. The greater trochanter forms the key lateral landmark, while the acetabular region lies medial and slightly anterior relative to the minimus belly. Gluteus minimus matters because it is not just a hip abductor, it is also a primary medial rotator and a dynamic stabilizer that controls pelvic drop during single-leg stance, acting in concert with gluteus medius. Weakness or tears can present with Trendelenburg gait and lateral hip pain, and its deep tendon on the greater trochanter is part of the gluteal tendinopathy spectrum often mislabeled as trochanteric bursitis. Surgeons and radiologists use this anatomy when planning and interpreting lateral hip MRI, and when selecting approaches that spare the abductors, since iatrogenic injury to the superior gluteal nerve can denervate minimus and medius proximal to their muscle bellies.
