Description
Arising from the anteromedial orbital floor of the maxilla, the inferior oblique sweeps posterolaterally beneath the globe as a slender, tapering fascicle before blending into its scleral insertion on the posterolateral aspect of the eyeball. In this lateral section through the skull, the muscle sits inferior to the inferior rectus and lateral to the lacrimal sac region, crossing the orbital fat just anterior to the optic nerve. The orbital walls, adjacent maxillary sinus, nasal cavity, and dentition of the maxilla provide hard-tissue landmarks, while orbital vessels and nerves track toward the orbital apex. Oblique muscle anatomy is easiest to teach when you can see the under-surface course of the inferior oblique relative to the globe and the orbital floor. That relationship matters clinically during inferior oblique weakening procedures (myectomy or recession) for overelevation in adduction, as well as in blowout fractures of the orbital floor where extraocular muscle entrapment and diplopia enter the differential. Oculomotor nerve (CN III) supply to the inferior oblique and its proximity to the infraorbital groove and canal region also frame discussions of post-traumatic neuropathic symptoms and peri-orbital surgical corridors. Small muscle, big consequences. Use this artwork in gross anatomy and neuroanatomy teaching to differentiate inferior oblique action from inferior rectus, or in ophthalmology and strabismus texts to support surgical planning diagrams that require accurate spatial relationships in a lateral orbital view.
