Description
Arising from the annulus tendineus communis at the orbital apex, the inferior rectus runs anteriorly along the orbital floor to insert on the inferior sclera, just posterior to the corneal limbus. From a lateral perspective within a skull section, its muscle belly sits inferior to the optic nerve and inferior ophthalmic vessels, with the inferior oblique coursing more anteromedially beneath the globe. The medial and lateral rectus muscles bracket the globe on either side, while the superior rectus lies superiorly within the muscle cone. Primary action is depression of the eye, but the inferior rectus also contributes to adduction and extorsion, so its line of pull is a reliable way to teach why downgaze becomes mechanically complex once the eye is already adducted. Clinically, this anatomy underpins restrictive strabismus in thyroid eye disease, where enlargement and fibrosis of the inferior rectus commonly produce limited elevation and a characteristic hypotropia. It also maps directly onto the pathways used in orbital decompression and fracture repair of the orbital floor, where scarring or entrapment can tether the inferior rectus and mimic a neurogenic palsy.
