Description
Arising from the common tendinous ring (annulus of Zinn) at the orbital apex, the inferior rectus muscle courses anteriorly along the orbital floor to insert on the inferior sclera, just posterior to the corneoscleral limbus. Its flattened belly sits inferior to the globe, bordered medially and laterally by the medial and lateral rectus muscles, while the inferior oblique passes inferolaterally beneath the globe toward the posterolateral orbit. A sectional skull view also brings the bony orbit into play, with the orbital plate of the maxilla forming most of the floor and the zygomatic and palatine contributions seen laterally and posteriorly. Functionally, inferior rectus acts as a primary depressor of the eye, with secondary adduction and extorsion, so its line of pull is central to explaining why vertical diplopia changes with gaze position. The muscle’s proximity to the orbital floor makes it a key structure in blowout fractures, where herniation of orbital contents and tethering of the inferior rectus can produce limitation of upgaze and an oculocardiac reflex. In strabismus surgery, the inferior rectus is a frequent target for recession in restrictive hypotropia and for thyroid eye disease related fibrosis, and the sectional perspective helps clarify why scarring and edema in the inferior orbit distort the globe’s resting position.
