Description
Sectioned male skull and orbit expose the globe within the bony socket, with the superior rectus muscle running along the superior aspect of the eyeball, deep to the levator palpebrae superioris and beneath the orbital roof formed primarily by the frontal bone. Posteriorly, the superior rectus tapers toward its tendinous origin at the common tendinous ring (annulus of Zinn) near the orbital apex, then courses anteriorly to insert on the superior sclera, just posterior to the corneoscleral limbus. The relationship to the optic nerve is implied at the apex, and the rectus path sits medial to the lacrimal fossa and lateral to the ethmoid sidewall. Clean spatial anatomy. This lateral cutaway makes the superior rectus easier to teach because it clarifies why elevation is rarely a pure movement: with the eye abducted, superior rectus acts as a strong elevator, but in primary gaze it couples elevation with adduction and intorsion, a frequent source of confusion in extraocular motility exams. Clinically, the superior rectus and levator complex matters in superior orbitotomy and in orbital decompression, where protecting the superior division of the oculomotor nerve and preserving the levator plane reduces postoperative diplopia and ptosis. Vertical diplopia after trauma or surgery often traces back to edema, entrapment, or scarring along this superior corridor.
