Seen from a superior perspective through a cranial section, the lateral rectus muscle occupies the lateral wall of the orbit and runs anteriorly from the common tendinous ring (annulus of Zinn) toward its insertion on the lateral sclera, just posterior to the corneoscleral junction. Medial to the muscle belly, the orbital apex region around the optic canal and superior orbital fissure is implied, with the globe seated centrally within the bony orbit. Portions of the other extraocular muscles are logically present around the eyeball, with the recti arranged in a cone that narrows posteriorly at the annulus and broadens anteriorly around the equator of the globe. Bony margins of the orbital roof and lateral wall frame the muscle’s course. This angle matters because it clarifies how the lateral rectus relates to the orbital apex, where crowding can become clinically relevant in thyroid eye disease, orbital apex syndrome, or traumatic hemorrhage. Abducens nerve (CN VI) palsy produces impaired abduction and horizontal diplopia, and a superior view helps you teach why even small changes in muscle length, stiffness, or pulley position can alter ocular alignment. The common tendinous ring is also a surgical and anesthetic landmark, relevant to retrobulbar blocks and to approaches that must respect the structures traversing the superior orbital fissure.
