Description
Arising from the common tendinous ring at the orbital apex, the inferior rectus courses anteriorly along the inferior surface of the globe to insert on the sclera just posterior to the corneoscleral limbus, inferior to the cornea and pupil. From this anterior perspective, the muscle belly sits inferior to the visual axis, with its tendon flattening as it approaches the anterior sclera; adjacent extraocular muscles are partially included at the medial and lateral margins, clarifying how the rectus group embraces the globe. The relationship to the inferior oblique is implied by the inferior quadrant location, even when that oblique belly is not the primary focus. Inferior rectus anatomy matters in two places clinicians routinely argue about: restrictive strabismus and orbital trauma. In thyroid eye disease, fibrosis of the inferior rectus limits elevation and produces hypotropia, and an anterior view helps explain why forced-duction testing feels tight when the eye is elevated. The same orientation also mirrors what surgeons see in an inferior transconjunctival approach for strabismus recession or resection, where the tendon’s insertion relative to the limbus guides exposure, suture placement, and avoidance of adjacent structures such as the inferior oblique and the anterior ciliary vessels. Small distances matter.
