Description
Seen in lateral profile beneath the skin, the internal oblique forms the intermediate layer of the anterolateral abdominal wall, lying deep to the external oblique and superficial to transversus abdominis. Its fleshy fibers arise from the thoracolumbar fascia posteriorly, the anterior two thirds of the iliac crest inferiorly, and the lateral half of the inguinal ligament, then course superomedially toward the costal margin and linea alba. Along the lower ribs, the muscle interdigitates with costal cartilages and blends anteriorly into a broad aponeurosis that contributes to the rectus sheath, while the iliac crest and pelvis provide the inferior bony framework in the same field. Lateral anatomy matters here because the internal oblique is a key landmark for groin and abdominal wall surgery, where the muscle’s arching lower fibers and their aponeurotic margins help define the inguinal canal region. The conjoint tendon (formed with transversus abdominis) reinforces the posterior wall medially, and weakness or disruption in this zone relates directly to direct inguinal hernia formation and recurrence after repair. It also frames regional anesthesia: the iliohypogastric and ilioinguinal nerves travel between internal oblique and transversus abdominis, the same fascial plane targeted in a transversus abdominis plane (TAP) block for lower abdominal incisions.
