Description
Presented in lateral profile with the skin rendered translucent, the lateral abdominal wall is opened to emphasize the internal oblique muscle as an intermediate layer between the external oblique superficially and the transversus abdominis deep. Its fibers course superomedially from the iliac crest and thoracolumbar fascia toward the inferior ribs and a broad anterior aponeurosis that contributes to the rectus sheath and linea alba. Superiorly the slips approach the costal margin, while inferiorly the lower fibers arc toward the inguinal region, where they relate to the inguinal ligament and may form part of the conjoint tendon medially. The rib cage and segments of the vertebral column provide bony landmarks behind the muscle. Clear orientation. Internal oblique anatomy matters because the transition from muscle belly to aponeurosis defines the layered planes surgeons and anesthetists depend on, including the transversus abdominis plane between internal oblique and transversus abdominis targeted for TAP blocks in abdominal wall analgesia. Inferomedially, its relationship to the inguinal canal floor helps explain patterns of direct inguinal hernia and why reinforcement in open repairs often references the conjoint tendon region. For teaching, the oblique fiber direction relative to the external oblique is the easiest cue for distinguishing layers on dissection and on cross sectional imaging.
