Description
Shown beneath the skin of an adult male trunk, the lateral abdominal wall is opened to expose the internal oblique muscle and its layered relationship to the external oblique superficially and the transversus abdominis deep to it. Fibers of the internal oblique run superomedially from the iliac crest and thoracolumbar fascia toward the inferior ribs and linea alba, crossing the inferomedial fiber direction of the external oblique. Posteriorly, the muscle’s origin from the deep thoracolumbar fascia is emphasized, while inferiorly its fibers contribute to the conjoint tendon and form the cremaster muscle around the spermatic cord via extensions into the inguinal canal. Internal oblique fibers and aponeurosis form part of the roof of the inguinal canal and, together with transversus abdominis, reinforce the posterior wall via the conjoint tendon, anatomy that underlies patterns of direct inguinal hernia and guides mesh placement and fixation planes during open repair. The cremasteric origin also explains why cord structures move with muscle contraction and why dissection too deep can enter the canal unintentionally.
