Description
Beneath the reflected skin of the male anterolateral abdominal wall, the internal oblique is presented as the intermediate muscular layer deep to the external oblique and superficial to transversus abdominis. Its fibers course superomedially from the iliac crest and thoracolumbar fascia toward the inferior borders of the lower ribs, while inferiorly they sweep toward the inguinal region where the broad sheet transitions into an aponeurosis. Medially, the internal oblique contributes to the rectus sheath adjacent to the linea alba, and inferomedially its lower fibers give rise to the cremaster muscle in the spermatic cord. Understanding this layer matters because the internal oblique helps define the inguinal canal anatomy that clinicians test daily and surgeons repair often, including the muscular arch formed with transversus abdominis above the spermatic cord. Its aponeurosis participates in the anterior wall of the rectus sheath above the arcuate line and, by splitting, forms both anterior and posterior laminae at mid-abdomen, a relationship that explains why hematomas and fascial plane spread can behave differently above versus below that level.
