Description
Seen from the lateral side of the ankle just deep to the skin, the superior extensor retinaculum forms a thick transverse band spanning from the distal anterior fibula laterally toward the distal tibia medially, lying superior to the malleolar prominences. Deep to it run the anterior compartment tendons as they cross the ankle joint: tibialis anterior most medially, extensor hallucis longus centrally, and extensor digitorum longus with fibularis tertius more laterally, all continuing distally toward the dorsum of the foot. The retinaculum sits superficial to these tendons and their synovial sheaths and blends with the crural fascia proximal to the tibiotalar joint line. Clear boundaries. This lateral perspective matters because the superior extensor retinaculum is the first pulley in the extensor tunnel system, preventing bowstringing of the long extensor tendons during ankle dorsiflexion and rapid gait. It also frames common points of irritation after inversion injuries and repetitive dorsiflexion, where tenosynovitis of extensor digitorum longus or extensor hallucis longus can present with pain and crepitus under the retinacular band. For surgical planning, the retinaculum and its relationship to the anterior ankle tendons help orient incisions for anterior ankle arthroscopy portals and for approaches to tendon sheath release while respecting the superficial course of the superficial fibular nerve branches in the lateral distal leg.
